血管内超声在冠心病介入诊疗中的临床应用研究

摘要：

目的:探讨血管内超声(IVUS)在冠心病介入诊疗应用中的临床价值。方法:36例确诊为冠心病的病人,其中男性28例,女性8例,采用先冠脉造影(CAG)然后行IVUS检查,PCI术后再次行IVUS,比较两者及PCI前后IVUS结果的差异。结果:与IVUS相比较,CAG不能很好判断临界病变、软斑块及纤维斑块,但在钙化斑块、自发性夹层及血栓方面有较高特异性;IVUS可及时发现PCI术后支架贴壁不良及支架膨胀不全等。结论:IVUS可精确判断CAG难以发现的冠脉病变并可指导PCI治疗,应予推广。

Objective:To investigate the clinical value of intravascular ultrasound in diagnosis and therapy in coronary artery disease.Methods:A total of 36 CAD patients were recruited.Coronary artery were examined by CAG and IVUS.Plaque characteristics(soft,fibrous,calcified and mixed plaques)and plaque burden at minimum area were detected and analyzed.The results of before PCI and after PCI derived from IVUS were compared.Results:42 vessels were examined by CAG and IVUS.CAG was not able to identify soft and fibrous plaques and fibrous cap,but could identify calcified plaques,spontaneous dissection,thrombosis and positive vessel remodeling better.IVUS could found ASM and stent inflation flubber promptly.Conclusions:IVUS could accurate determine coronary lesions which CAG could not determine and could give directions to PCI results.
