早期药物联合心理干预治疗脑卒中后抑郁患者的临床研究

摘要：

目的探讨早期药物联合心理干预对脑卒中后抑郁症状和神经功能恢复的影响。方法选取80例脑卒中后抑郁患者，经汉密顿抑郁量表（HAMD\|17）评分≥8分，随机分成治疗组和对照组各40例，两组均予脑卒中常规治疗和康复训练，治疗组在此基础上加用药物(帕罗西汀)并联合心理干预治疗。在治疗前、治疗后2、4、8周分别对患者行HAMD\|17和神经功能缺损程度（SSS）评分。结果治疗后4、8周治疗组患者HAMD\|17和SSS评分明显优于对照组（P＜0.01）。抑郁症状和神经功能临床疗效治疗组明显优于对照组（P＜0.01或P＜0.05）。结论早期药物联合心理干预可明显改善脑卒中后抑郁程度，并能促进神经功能恢复。

ObjectiveTo explore the effect of early medicine and mental intervention for depression syndrome and recovery of neurological function in post\|stroke depression patients.Methods80 patients of post\|stroke depression were randomly divided into the treatment group and the control group.Both groups were all received routine treatment and rehabilitation of stroke,and the treatment group was given extra paroxetine and mental intervention.The HAMD\|17 and SSS were observed before treatment and at the ends of 2nd,4th,8th week after treatment.ResultsThe HAMD\|17 and SSS of the treatment group were better than the control group(P＜0.01) at the 4th,8th week after treatment.The clinical therapeutic effect and depression syndrome of the comprehensive treatment group were improved significantly(P＜0.01 or P＜0.05).ConclusionsThe early combination therapy of mental intervention and paroxetine can significantly improve the level of post\|stroke depression,and promote the recovery of neurological function.
