腹腔镜辅助下阴式子宫肌瘤剔除术的临床应用价值

摘要：目的 探讨腹腔镜辅助下阴式子宫肌瘤剔除术的临床应用价值。方法分析2006年7月至2008年7月我院120例行子宫肌瘤剔除术患者，其中采用腹腔镜辅助下阴式子宫肌瘤剔除术60例，腹腔镜下子宫肌瘤剔除术 30例，阴式子宫肌瘤剔除术30例。对三组患者的术中、术后相关资料进行分析比较。结果腹腔镜辅助组具有手术时间短,术中出血量少、手术前后HGB差值大，术后病率低等特点，差异显著,(P＜005),但腹腔镜组较其他两组术中出血量多，手术时间长，术后肌瘤复发率高，差异显著，(P＜005)。结论腹腔镜辅助下阴式子宫肌瘤剔除术不仅具有腹腔镜手术及阴式手术共同的优点,而且较腹腔镜手术减少术中出血，缩短手术时间，减少术后复发，又避免了阴式手术处理附件较困难、对盆腔情况缺乏全面的评估等弊病,是一种更为理想的微创手术方式，值得临床推广应用。

Abstract: Objective To investigate the clinical application of laparoscopic aid transvaginal myomectomyMethods From July 2006 to July 2008,60 cases were subjected to laparoscopic aid transvaginal myomectomy，in the meantime，30 cases were subjected to laparoscopic myomectomy,30 cases were subjected to transvaginal myomectomyThe perioperative and postoperative related material of all patients were analyzed and comparedResults Aparoscopic aid transvaginal myomectomy group had shortter operative time,less bleeding during operation,bigger differences in hemoglobin count before and after operation,lower rate of postoperative disease etc,All were statistically significant differences(P＜005),But laparoscopic myomectomy group had more bleeding during operation,longer operative time,higher recurrence rate after surgeryAll were statistically significant differences(P＜005)Conclusion laparoscopic aid transvaginal myomectomy not only had common advantages in the laparoscopic myomectomy and transvaginal myomectomy,but also had less bleeding,shortter operative time,lower disease rate than laparoscopic myomectomy;and avoided the lack of comprehensive assessment for annex and pelvic situationlaparoscopic aid transvaginal myomectomy is a more ideal minimally invasive surgery and worthy of clinical application.
