腹腔镜胆囊癌切除术治疗早期胆囊癌的临床效果和对患者生存期的影响观察

摘要：目的探讨腹腔镜胆囊癌切除术治疗早期胆囊癌的临床效果和对患者生存期的影响。方法对2007年5月至2011年11月在我院行腹腔镜胆囊癌切除术（观察组）的36例早期胆囊癌患者和行根治性胆囊癌切除术（对照组）的32例早期胆囊癌患者的临床效果及术后1、3、5年生存率进行比较分析。结果观察组术后感染发生率为111%、5年内复发率为806%，对照组术后感染发生率为750%、5年内复发率为531%，观察组术后感染发生率显著低于对照组而5年内复发率显著高于对照组，差异有统计学意义（P＜005）；观察组术后1、3、5年生存率分别为556%（20/36）、417%(15/36)、306%(11/36)，对照组术后1、3、5年生存率分别为938%（30/32）、781%（25/32）、594%(19/32)，对照组术后1、3、5年生存率均高于观察组，差异有统计学意义（P＜005）。结论外科手术治疗早期胆囊癌，应根据患者病情选择不同的手术方式，以达到既获得较好的近期临床效果又延长患者生存期的目的。

Abstract:Objective To investigate the clinical effects and quality of life in patients with early gallbladder cancer used Laparoscopic cholecystectomyMethods36 patients undergoing laparoscopic cholecystectomy(observation group)and 32 patients with radical cholecystectomy(control group) with early gallbladder cancer in our hospital from May 2007 to November 2011 were chosenThe clinical efficacy and postoperative 1,3,5\|year survival rates in the two groups were taken for analysisResultsThe rate of postoperative infection in the observation group was 111%The rate of recurrence in 5 years was 8056%The rate of postoperative infection in the control group was 75%The recurrence rate in 5\|year was 531%The rate of postoperative infection in the observation group was lower than that in the control group and the recurrence rate in 5 year was higherThe difference was statistically significant(P＜005)The survival rate of 1\|year,3\|year and 5\|year after operation in the observation group were 556%（20/36）、417%(15/36)、306%(11/36)And those in the control group were 938%（30/32）、781%（25/32）、594%(19/32)The difference was statistically significant(P＜005)ConclusionFor better clinical efficacy and prolong survival,different patients should be taken with different surgical treatment based on the condition
