应用拉米夫定治疗时间与HBV YMDD基因变异情况的分析

摘要：目的探讨慢性乙型肝炎（CHB)患者应用拉米夫定治疗时间长短与HBV YMDD基因变异情况的关系。方法将254例CHB患者按应用拉米夫定治疗时间不同分为＜24周、24~48周、49~96周和＞96周共4组，采用PCR荧光法检测血清HBV DNA水平和HBV YMDD基因变异情况。结果拉米夫定治疗时间长短与血清HBV DNA阳性率有相关性，随着拉米夫定治疗时间的延长，血清HBV DNA的阳性率逐渐上升。拉米夫定治疗时间长短与HBV DNA阳性患者的HBV YMDD基因变异率有关，随着拉米夫定治疗时间的延长，HBV DNA阳性患者的HBV YMDD基因变异率呈升高趋势，且拉米夫定治疗时间＞24周后HBV YMDD基因变异率显著升高。结论长时间应用拉米夫定治疗CHB可引起HBV YMDD基因变异，致使患者产生耐药。

Abstract:ObjectiveChronic hepatitis B(CHB)patients with lamivudine treatment duration and relationship of HBV YMDD.Methods254 patients with lamivudine treatment inpatients with CHB is divided into different time＜24 weeks、24~48 weeks、49~96weeks and ＞96 week a total of four groups,Using fluorescence PCR method for detection of serum levels of HBV DNA and HBV YMDD mutation.ResultsThere is a correlation between the positive rate of lamivudine treatment duration and serum HBV DNA,with the extension of lamivudine treatment duration,the positive rate of serum HBV DNA increased gradually.HBV YMDD gene mutation of lamivudine treatment durationis related tothe HBV DNA positive rate of patients,with the extension of lamivudine treatment duration,HBV YMDD gene mutation of HBV DNA positive rate of patients is increasing,and Lamivudine treatment duration＞HBV YMDD mutation rate after 24 weeks significantly increased.ConclusionCHB lamivudine therapy for a long time may cause HBV YMDD mutation,causingpatients’resistance.
