腓骨瓣重建下颌骨时受区血管的选择

摘要：

摘要：目的分析腓骨瓣修复重建下颌骨时受区血管的选择情况及效果。方法回顾收集2005—2012年间我院腓骨瓣游离移植重建下颌骨的患者共32例，分析32例患者面颈部受区动静脉的选择及腓骨瓣存活情况。结果32例腓骨瓣吻合动脉32根，受区动脉包括颌外动脉27根，甲状腺上动脉1根，舌动脉3根，颈外动脉1根；吻合静脉36根，受区静脉包括面静脉11根，颈外静脉11根，面总静脉9根，面后静脉3根，颈内静脉2根。术后3例出现血管危象，经抢救后均成活。结论游离腓骨瓣重建下颌骨时，面颈部有多个动静脉可供选择，可根据受区血管管径、走行、管腔压力等因素综合考虑。

Abstract:ObjectiveAnalysis the choice of the vessels and therapeutic effect in mandibular reconstruction using fibular flaps.Methods32 collected patients undergone mandibular reconstruction using fibular flaps from 2005 to 2012.Analysis the choice of the recipient vessel in head and neck and observe the living of the flaps.ResultsIn our investigate,overall,32 arteries and 36 veins were involved,which included 27 facial arteries,1 superior thyroid artery,3 lingual arteries and 11 facial veins,9 common facial veins,3 retromandibular veins,11 external carotid veins,2 internal carotid vein.3 vessel crisises had been observed,and all had been reserved when received salvage in time.ConclusionIn mandibular reconstruction using free fibular flap,there are several arteries and veins could be involved in head and neck.Diameter,location and the pressure of the vessel need to be considered.Key Words:Fibula flaps;Mandibular;Reconstruction；Recipient vessel
