胰岛自身抗体检测对筛查成人隐匿性自身免疫糖尿病的临床意义

摘要：

目的通过对血清谷氨酸脱羧酶抗体(GADA)、胰岛细胞抗体(ICA)和胰岛素自身抗体(IAA)3种胰岛自身抗体联合检测，探讨2型糖尿病（T2DM）患者中筛查成人隐匿性自身免疫糖尿病（LADA）的临床意义。方法测定200例诊断T2DM患者GADA、ICA、IAA、BMI、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL\|C)、高密度脂蛋白（HDL\|C）、空腹C肽、餐后2 h C肽、糖化血红蛋白(HbA1c)。检测出其中的LADA患者，并与T2DM患者比较临床指标的差异。结果200例T2DM患者中共确诊为LADA患者15例（75％）；与自身抗体阴性组相比，自身抗体阳性组BMI、TC、空腹C肽、餐后2 h C肽明显降低，而HbA1c明显增高，差异有统计学意义（P＜005）。结论GADA、ICA、IAA联合检测对筛查LADA有重要指导价值，各项临床指标联合胰岛自身抗体检测可提高LADA患者的检出率。

Abstract:ObjectiveTo explore the clinical significance of combined test of glutamic acid decarboxylase antibody(GADA)、islet cell antibody (ICA)and insulin autoantibody (IAA)in screening latent autoimmune diabetes in adults(LADA） from type 2 diabetes(T2DM)MethodsIn 200 T2DM patients，GADA，ICA，IAA，BMI，HbA1c，TC，TG，LDL\|C，HDL\|C，fasting C\|peptide and C\|peptide of 2\|hours post\|meal were tested to find out the patients with LADA，whose various indexes were compared with the patients of T2DMResultsIn 200 cases of T2DM，15 cases were detected LADA（75％），compared with islet autoantibodies negative patients，BMI、TC、fasting C\|peptide and C\|peptide of 2\|hours post\|meal were lower，but HbA1c was higherThere were statistical differences in two group（P＜005）ConclusionThe combined test of GADA，ICA，IAA has important guiding valueThe combination of clinical indexes and islet autoantibodies can increase detection rate of LADA
