HPV分型检测在子宫颈病变诊断中的价值

摘要：目的探讨HPV分型检测在宫颈病变诊断中的临床价值。方法对2013年9月至2014年9月收治的80例宫颈病变患者行液基薄层细胞检测（TCT）和HPV分型检测，并对其检测进行分析。结果80例患者中，TCT结果示ASC53例、LSIL20例、HSIL7例；HPV分型检测示HPV阳性49例，阳性率613%。检出的基因型有15种，分别是高危型HPV16、18、31、33、39、45、52、58、35、56和低危型HPV6、11、42、44及亚型CP8304；其中高危型HPV感染28例（占571%）、低危型HPV感染10例（占204%）、混合型HPV感染11例（占224%）。TCT结果与HPV检测阳性结果呈正相关，即随着TCT显示宫颈病变严重程度的增高，HPV检测阳性率亦上升；HPV检测阴性31例中，仅5例病理确诊为CIN。结论HPV分型检测在临床诊断宫颈病变中具有极高的灵敏度，其检测结果的阴性预测值极高，值得推广使用。

Abstract:ObjectiveTo study the clinical value of HPV typing test for diagnosis of cervical lesionsMethods80 patients with cervical lesions in our hospital from September 2013 to September 2014 were received TCT and HPV typing test,the results were analyzed.ResultsTCT results showed 53 ASC cases,20 LSIL cases and 7 HSIL casesAccording to HPV\|DNA typing test,the positive rate of HPV was 613%(49 cases);15 gene types were detected,including HR\|HPV(HPV16,18,31,33,39,45,52,58,35,56),LR\|HPV(HPV6,11,42,44)and the common type of CP8304;there were 28 HR\|HPV patients(571%),10 LR\|HPV patients(204%)and 12 hybrid\|HPV patients(224%)TCT results were positively correlated with HPV typing test results,that was to say the positive rate of HPV was increased with the severity of lesion showed by TCT;there were only 5 cases pathological diagnosis as CIN in the 31 cases of negative HPVConclusionHPV typing test can provide the flexible clinical diagnosis for cervical lesions and has a higher predictive value on the negative resultsIt is worthy of clinical promotion and applicationKey Words:Cervical lesion;HPV typing test;Clinical diagnosis
