Abstract：Objective To observe the effect of laryngeal ventilation-general anesthesia combined with epidural anesthesia on emergence agitation and stress response in elder patients with radical rectal cancer operation. Method 60 patients with≥65 years，ASA Ⅰ-Ⅱ and elective radical resection of rectal cancer were selected and randomly divided into two groups according to different anesthesia：Laryngeal ventilation-general anesthesia combined with epidural anesthesia group（group A）and general anesthesia group（group B），30 cases in each group. The same general anesthesia induction drugs were used in the two groups. Recorded sedation and sedation score at each point in the time that was before the start of anesthesia（T1），pull out the mask before the mask（T2），1 min after pulling out the mask（T3）and 5 min after pulling out the mask（T4），plasma cortisol and blood glucose concentrations and MAP and HR value. Awaken time，laryngeal removed time，amount of Sevoflurane and Remifentanil were recorded at the same time. Results At the time of T2，T3 and T4，the emergence agitation，Ramsay score，blood concentration of plasma cortisol，glucose，the value of MAP and HR，the amount of Sevoflurane and Remifentanil of group A were significantly lower than those of group B. The difference between the two groups was statistically significant（P＜0.05）. Conclusion Laryngeal ventilation-general anesthesia combined with epidural anesthesia for elderly patients with rectal cancer radical surgery can provide a stable hemodynamics，reduce emergence agitation and stress response significantly during the recovery period，and improve the wake quality of anesthesia.
