应用三维斑点追踪技术评价川崎病患儿左心室收缩功能

摘要:目的 应用三维斑点追踪技术(Three-dimensional speckle tracking imaging, 3D-STI)评价川崎病(Kawasaki disease,KD)患儿早期左心室的收缩功能。方法 以川崎病患儿(KD组)58例作为研究对象,40例健康儿童为对照组。应用常规超声检测左室收缩功能,应用3D-STI检测左室整体的纵向、环向、径向和面积收缩期峰值应变,并进行结果比较。结果 常规超声检测心脏的整体收缩功能指标两组间差异均无统计学意义(均P>0.05);3D-STI检测得到KD组患儿整体纵向应变及面积应变的收缩期峰值应变低于对照组,组间差异有统计学意义(P<0.05)。结论 3D-STI可以检测出KD患儿早期左心室心肌收缩运动的改变,可用于KD患儿的随访,以监测心脏功能变化。

Abstract:Objective To evaluate the systolic function of left ventricle in children with Kawasaki disease (KD) by three-dimensional speckle tracking imaging (3D-STI). Methods 58 children with KD were as studied group and 40 healthy children as control group. The left ventricular systolic function was measured by conventional ultrasound, and the whole longitudinal, circumferential, radial and regional systolic peak strain of the left ventricle were measured by 3D-STI, the results in two groups were compared. Results There was no significant difference between the two groups in the systolic function by conventional ultrasound (P>0.05). The whole longitudinal strain and area strain peak systolic strain measured by 3D-STI in KD group was lower than that of the control group, the difference was statistically significant (P<0.05). Conclusion 3D-STI can detect the change of the left ventricular systolic movement in children with KD, and can be used to monitor the changes of cardiac function in children with KD.
