Abstract：Objective To explore the feasibility of blood smear Miller disc platelet counts（Miller??s method）as a  reexamination method for thrombocytopenic. Methods 121 venous anticoagulated blood samples with thrombocytopenia tested by hematology analyzer were collected and all blood smears were prepared and stained. Counting erythrocytes in the small square and the platelets in the large square under the microscope, the number of platelet were calculated according to the platelet/RBC ratio and the absolute erythrocyte. Miller's reproducibility was checked for a sample. The correlation and consistency between the Miller count and fluorescent platelet count（PLT-F）were analyzed for other 120 samples. The number of thrombocytopenia samples for reexamination in the laboratory within six months was investigated. Results Satisfactory precision were obtained when microscopic platelet counting were greater than 100.The coefficient of variation of Miller count method was 3.83%. The correlative coefficients between Miller??s and PLT-F??s method was 0.987, and the confidence interval was 0.982～0.991. Bland-Altman analysis showed Miller and PLT-F had consistency. The accounted ratio of thrombocytopenic samples was about 5.8% and the review rate of blood smear needed to estimate was about 2.0% of the specimens in the same period within half a year. Conclusion Thrombocytopenic samples are common in clinical laboratory. The repeatability of the Miller platelet count on blood smear is intuitive and simple, it has a good correlation and consistency with the results of hematology analysis, which can be used for the review of thrombocytopenic samples.
