胰岛素治疗妊娠期糖尿病患者妊娠结局的临床探讨

摘要：目的探讨采用胰岛素治疗妊娠期糖尿病患者的并发症和围产儿结局。方法选择2016年1月～2020年1月于我院治疗的186例采用胰岛素治疗的妊娠期糖尿病患者，根据控制血糖标准将其分为A组（血糖稳定组）112例和B组（血糖不稳定组）74例，以及同期正常妊娠孕妇为C组178例，观察3组妊娠并发症及围产儿结局。结果 3组妊娠并发症和围产儿结局比较，A组与B组比较差异均有统计学意义（均P＜0.05）；与C组比较差异均无统计学意义（均P＞0.05）。结论妊娠期糖尿病胰岛素治疗对胎儿是安全的，血糖控制稳定患者的孕期情况和围产儿结局与正常孕妇相同，故应重视胰岛素对控制妊娠期糖尿病患者血糖的作用，尽早干预和治疗。

Abstract：Objective To explore the pregnancy complications and perinatal outcomes of gestational diabetes treated with insulin. Methods 186 patients with gestational diabetes treated with insulin in our hospital from January 2016 to January 2020 were selected and divided into group A（stable blood sugar group）with 112 cases and group B（unstable blood sugar group）with 74 cases and 178 cases of normal pregnant women in the same period were in group C according to blood glucose control standards. Pregnancy complications and perinatal outcomes in 3 groups were observed. Results Comparing pregnancy complications and perinatal outcomes in the three groups, the differences between group A and group B were statistically significant （all P＜0.05）; there were no significant differences compared with group C（all P＞0.05）. Conclusion Insulin for gestational diabetes is safe for the fetus, the pregnancy conditions and perinatal outcomes of patients with stable blood glucose control are the same as those of normal pregnant women. Therefore, attention should be paid to the effect of insulin on blood glucose control in patients with gestational diabetes, and intervention and treatment should be carried out as soon as possible.
