上海市月浦社区老年2型糖尿病患者视网膜病变与糖化血红蛋白等检测指标的相关性研究

摘要：目的探讨2型糖尿病（T2DM）患者视网膜病变（DR）与糖化血红蛋白（HbA1c）、空腹血糖（FPG）、尿微量白蛋白（MA）水平的关系。方法选择2017年3—9月上海市月浦社区老年体检患者245例，根据眼底检查是否发生糖尿病视网膜病变将其分为糖尿病视网膜病变（DR）组116例与无视网膜病变（NDR）组129例，回顾性分析两组糖尿病及DR相关化验指标水平。结果 DR组HbAlc、FPG、MA/UCRE均高于NDR组，组间比较差异均有统计学意义（χ2＝3.06，t＝3.48、3.82，均P＜0.01）；DR组MA水平高于NDR组，组间比较差异有统计学意义（t＝2.54，P＜0.05）；高水平的HbA1c是糖尿病视网膜病变发生的独立危险因素（OR＝1.389，P＜0.05）。结论 DR患者视网膜病变与HbAlc存在相关性，临床应积极关注糖尿病患者的HbA1c、FPG、MA、MA/UCRE比值水平，注意视网膜病变的早期筛查并积极干预，可降低DR的发生。

Abstract：Objective To explore the relationship between diabetic retinopathy（DR）and the level of glycosylated hemoglobin（HbA1c）, fasting blood glucose（FPG）and urine microalbumin（MA）in patients with type 2 diabetes mellitus（T2DM）. Methods From Mar. to Sep. 2017, 245 patients in the Yuepu community were selected and were divided into DR group（116 cases）and NDR group（129 cases）by the results of the fundus examination, the laboratory indicators of DR and diabetes mellitus were retrospective analyzed. Results HbAlc, FPG, MA/UCRE in the DR group were obviously higher than the patients in the NDR group, the differences between groups were statistically significant（χ2＝3.06, t＝3.48,3.82, all P＜0.01）; MA level of the DR group was higher than the NDR group, the difference was statistically significant（t＝2.54, P＜0.05）; The high levels of HbA1c were an independent risk factor for the occurrence of DR（OR=1.389, P＜0.05）. Conclusion There is a correlation between retinopathy and HbA1c in patients with DR; We should pay more attention to HbA1c, FPG, MA, MA/ UCRE in diabetic patients, early screening of retinopathy and actively intervene to reduce the incidence of DR.
