超声引导下经皮球囊扩张在血液透析患者动静脉内瘘狭窄中的临床应用*

摘要：目的探讨超声引导下经皮球囊扩张在血液透析患者动静脉内瘘狭窄中的临床应用价值。方法选择2019年7月—2020年9月我院血液透析患者中出现动静脉内瘘狭窄的67例患者为研究对象（自体动静脉内瘘狭窄59例、人工血管动静脉内瘘狭窄8例），均行超声引导下经皮球囊扩张治疗。结果手术成功66例。1例因吻合口增生明显，导丝通过困难致手术失败，改为重新手术建立动静脉内瘘。手术成功率为98.5%。术后随访3～15个月，3例在术后半年出现再次狭窄，予以再次球囊扩张后恢复动静脉内瘘流量，其余64例未出现狭窄及内瘘流量减少。结论超声引导下球囊扩张治疗动静脉内瘘狭窄安全、有效，能够有效避免DSA下球囊扩张介入治疗的缺点及DSA高昂的投入，适合在基层医院推广。

Abstract：Objective To cinvestigate the clinical value of ultrasound-guided percutaneous balloon dilatation in the treatment of arteriovenous fistula stenosis in hemodialysis patients. Methods From July 2019 to September 2020, 67 hemodialysis patients with arteriovenous fistula stenosis（59 patients with autogenous arteriovenous fistula stenosis and 8 patients with artificial arteriovenous fistula stenosis）were selected as research subjects and all treated with ultrasound-guided percutaneous balloon dilatation. Results The operation was successful in 66 patients. One patient failed due to the obvious anastomotic hyperplasia and difficulty in passing the guide wire, and reoperation was performed to establish an arteriovenous fistula. The surgical success rate was 98.5%. Follow-up for 3 to 15 months after operation, 3 cases developed restenosis half a year after operation, and the flow of arteriovenous fistula was restored after balloon dilation. The operation was changed to establish arteriovenous fistula again. The remaining 64 cases had no stenosis and flow decreased of internal fistula. Conclusion Ultrasound-guided balloon dilation for arteriovenous fistula stenosis is safe and effective, and can effectively avoid the short comings and high cost of DSA balloon dilation intervention. It is suitable for promotion in primary hospitals.
