铒激光联合牙周基础治疗对胃Hp感染慢性牙周炎患者的作用*

摘要：目的探讨铒激光联合牙周基础治疗对胃Hp感染慢性牙周炎患者牙周临床指数、龈沟内炎症因子及胃Hp阳性复发率的影响。方法选取2020年3月—2021年9月在包头医学院第一附属医院口腔科及消化内科治疗的患者80例，均为近期结束抗Hp四联疗法治疗且复查13C-尿素呼气试验胃Hp阴性的慢性牙周炎患者，将其随机分为实验组与对照组，每组各40例。对照组接受牙周基础治疗，实验组在对照组基础上联合铒激光治疗。治疗后8周，比较两组牙周袋探诊深度（PD）、附着丧失（AL）、龈沟出血指数（SBI）和龈沟内白细胞介素1β（IL-1β）、白细胞介素6（IL-6）水平。结束治疗后第1、6个月，比较两组胃Hp的阳性复发率。结果对照组与实验组治疗后的PD、AL、SBI均较治疗前显著降低，组内比较差异均有统计学意义（t＝19.008、26.494、19.113、20.321、15.945、14.117，均P＜0.05）；且实验组治疗后的PD、AL、SBI均低于对照组，组间比较差异均有统计学意义（t＝4.770、2.633、2.545，均P＜0.05）。对照组与实验组治疗后的IL-1β、IL-6均较治疗前显著降低，组内比较差异均有统计学意义（t＝16.786、18.494、49.460、36.393，均P＜0.05）；且实验组治疗后的IL-1β、IL-6均低于对照组，组间比较差异均有统计学意义（t＝8.592、3.716，均P＜0.05）。治疗结束后1个月，两组胃Hp阳性复发率比较，差异无统计学意义（χ2＝0.392，P＞0.05）；治疗结束后6个月，实验组胃Hp阳性复发率低于对照组，组间比较差异有统计学意义（χ2＝4.528，P＜0.05）。结论　铒激光联合牙周基础治疗能够更有效地改善胃Hp感染的慢性牙周炎患者的牙周临床指数，降低龈沟内炎症因子水平，降低胃Hp阳性复发率，对疾病转归更有益。

Abstract：Objective To investigate the effects of Erbium laser combined with periodontal basic treatment on periodontal clinical index, inflammatory factors in gingival sulcus and recurrence of Hp in chronic periodontitis patients with gastric Hp infection. Methods A total of 80 patients with chronic periodontitis treated in the department of stomatology and gastroenterology of The First Affiliated Hospital of Baotou Medical College from March 2020 to September 2021 were selected, who had just finished the treatment of anti Hp quadruple therapy and had negative gastric Hp in 13C urea breath test were randomly divided into observation group and control group. The control group only received periodontal basic treatment, and the observation group received Erbium laser combined with periodontal basic therapy. The periodontal pocket probing depth （PD）, attachment loss（AL）, gingival sulcus bleeding index（SBI）and concentrations of inflammatory factor IL-1β and IL-6 in gingival sulcus were compared between the two groups in 8 weeks after treatment. At the first and sixth months after treatment, the recurrence rate of Hp in the two groups was compared. Results The PD, AL and SBI of the control group and the experimental group after treatment were significantly lower than those before treatment, and the differences within the groups were statistically significant（t＝19.008, 26.494, 19.113, 20.321, 15.945, 14.117, all P＜0.05）; And the PD, AL and SBI in the experimental group after treatment were lower than those in the control group, and the differences between the groups were statistically significant（t＝4.770, 2.633, 2.545, all P＜0.05）. Conclusion Compared with simple periodontal basic treatment, Erbium laser combined with periodontal basic treatment can significantly improve the periodontal clinical index, reduce the level of inflammatory factors in gingival sulcus and improve the recurrence after radical gastrectomy for Hp in patients with chronic periodontitis and gastric Hp infection, which is more beneficial to the prognosis of the disease.
